Client acknowledges and understands that, although Advocate or one or more of Advocate's employees may be licensed to practice medicine, Advocate is not providing medical services or advice to Client and that nothing stated during the course of any consultation or other communication between Client, Advocate and/or any employee of Advocate should be construed as providing medical services or advice of any kind or nature whatsoever. Specifically, the term "practicing medicine" shall be defined as:
"the diagnosis, treatment, or offer to treat a mental or physical disease or disorder or a physical deformity or injury by any system or method, or the attempt to effect cures of those conditions, by a person who:
a) publicly professes to be a physician or surgeon; or
b) directly or indirectly charges money or other compensation for those services.
Client acknowledges and agrees that the services and information provided by Advocate are not a substitute for the advice of Client's primary physician or any other medical professional and are not intended in any way to diagnose or treat any illness or disease or to interpret any information with respect to any symptoms pertaining to any illness or disease.
CLIENT FURTHER UNDERSTANDS THIS IS NOT INSURANCE, NOR IS IT INTENDED TO REPLACE INSURANCE. This is a patient advocacy program only and may be discontinued or modified at any time. This is not covered by any life and health guarantee association.
CLIENT FURTHER UNDERSTANDS THAT IF CLIENT HAS AN EMERGENCY MEDICAL NEED THAT REQUIRES IMMEDIATE ATTENTION, CLIENT IS TO DIAL 911 OR SEEK ASSISTANCE AT THE NEAREST EMERGENCY ROOM.
